After 27 years of working in managed Medicaid and Medicare in Arizona and several other states I suspect when the smoke clears the Federal Government will spend as much if not more dollars on Medicare and Medicaid as the have historically. The reason is system accountibility and individual member accountibility. When those two acountibilities do not exist, no savings will occur.
Please do not misunderstand, I love the concept, I just do not believe the participating organizations understand how to make it happen.
